- Application for registration of the birth certificate (MODUL2 – caso 2), duly completed and signed by both parents;
- Self-declaration form pursuant to Presidential Decree 445/2000, duly completed and signed;
- Minor’s birth certificate (for the United Kingdom: full birth certificate or certified copy of an entry of birth), duly legalized (*SEE BOX TRANSLATIONS AND LEGALIZATIONS);
- Translation of the birth certificate into Italian – the Apostille does not need to be translated (*SEE BOX TRANSLATIONS AND LEGALIZATIONS);
- Photocopy of the parents’ passports or valid identity documents;
- Photocopy of the minor’s foreign passport (if already available);
- If available, photocopy of previous identity documents of the Italian parent issued during their period of residence in Italy.
The Consulate will carry out the necessary checks on the declarations made in the Self-declaration form by requesting a historical certificate of residence from the Municipality/Municipalities where the parent resided as an Italian citizen for more than two years.
Please note that the processing time of the application also depends on the response times of the Municipalities.
