Please submit the below-listed documentation:
- APPLICATION FORM No. 5 (editable pdf) duly completed with all requested information (name LAST NAME of applicant, place and date of the death) and signed.
Please provide a legible email address where the Consulate will send a copy of the communication transmitted to the Municipality via certified email (PEC). Moreover, please provide a postal address to be used in case incomplete and/or incorrect documentation needs to be returned to the sender;
- ORIGINAL (not photocopy) of the “Certified Copy of an Entry of Death” issued by the competent Registry office;
- PAPER APOSTILLE issued by the “Foreign and Commonwealth Office – FCO” which certifies the signature of the Registrar on the document. An Apostille on the signature of a Notary Public and/or a solicitor is NOT For further information on how to get your document legalised, please click here;
- TRANSLATION into Italian ONLY of the death certificate – NOT OF THE APOSTILLE.

The translation may also be carried out independently but it will be accepted only if compliant with the original, complete with all the information and typewritten. A template of the translation is available here.
- PHOTOCOPY ONLY of the page with photo and signature of the deceased.
- PRESTAMPED SPECIAL DELIVERY ENVELOPE which should include the FULL name, postal address in the UK to be used for the return of any incomplete and not-compliant documentation to the sender.
