HOW TO ADD MARRIED SURNAME:

  • An Italian woman who is legally married, can retain her maiden name in her passport and other IDs. However, she may choose (but it is not mandatory) to have her married surname on the passport. An annotation on page 4 of her passport will be added. In order to add the married surname, the marriage certificate must already have been transcribed with this Consulate or with the Italian Autorities.
If the marriage has not yet been registered, it is necessary to first request the transcription of the marriage certificate through the Civil Registry Office of this Consulate following these instructions.
