How to apply for a passport – for Italians who are permanently unable to travel

Biometric data capture, in particular fingerprinting, is a prerequisite for the issue of an Italian passport.

The only exception allowed is if the applicant suffers from a medical condition which renders them permanently unable to travel.

Please note that the inability to travel must be permanent, not temporary.

 


 

To apply for a passport without fingerprints applicants, you must submit:

  • In the case of an adult over 18 y.o.: MOD1 printed and signed;
  • In the case of a minor between 12-17 y.o.: MOD2 signed by both parents including the photocopy of the parents’ passports. If one parent is not european, his/her signature must be legalized;
  • In the case of a minor between 0-11 y.o.: MOD3 signed by both parents including the photocopy of the parents’ passports. If one parent is not european, his/her signature must be legalized;
  • In the case of a minor between 0-17 y.o.: AFT2;
  • A medical certificate issued by a hospital or by a GP certifying that the applicant is “PERMANENTLY UNABLE TO TRAVEL”. Any certificate with a different wording will not be accepted;
  • 3 recent identical passport-size photos (taken within six months) compliant with the international ICAO standards (white background only);
  • Previous original passport, if it’s expired or deteriorated or, if not available, photocopy of national ID card or photocopy of a foreign passport. Driving licence or Oyster card are not accepted;
  • In case of lost / stolen passport, British Police online report and the relevant Italian lost/stolen passport form DEN1;
  • In case this is the first Italian passport, you need to submit a photocopy of the Italian ID card or a photocopy of the foreign passport. Driving licence or Oyster card are not accepted;
  • In case of passport expired more than 10 years ago, photocopy of a foreign passport or a photocopy italian ID card. Driving licence or Oyster card are not accepted;
  • POSTAL ORDER made payable to “Italian Consulate General”. Check the correct amount on our website at the Consular fees section;
  • Prepaid Special Delivery envelope (up to 500g) purchasable at any post office for the return of both the new and the old passport. We recommend to make a note of the tracking number for future reference.
