Passport for a minor aged 0 to 11 (only by post)

PREREQUISITES

  • The minor must be registered in the London AIRE catchment area.
  • The minor’s birth certificate must already have been registered in Italy.
    • If your child was born in Italy, the birth is automatically registered.
    • If your child was born abroad, you must ensure that the birth has already been transmitted/registered by the Consulate or by the Municipality in Italy. If not, please follow the procedure on our website and wait for confirmation before submitting the passport application.

 PASSPORT ISSUANCE

  • The application must be sent by post, attaching all the documents listed below. Once processing is complete, the Consulate General will send the passport to the address provided by the applicant on the prepaid envelope included with the application.
  • Processing time is approximately 5 weeks from the date your application is received by the Consulate (these timeframes are indicative and subject to change).

The receipt of payment must be printed and sent together with the documentation requested below. Applications without proof of payment will be returned to the sender.


REQUIRED DOCUMENTATION (CHECKLIST)

  1. Application form: MOD3 completed in block capitals and signed by both parents (signatures must match those on the passport/identity document attached to the application). Please ensure that you fill in all the required fields (including height and eye colour), as any missing information may cause delays.
  2. Legalisation (authentication) of the minor’s photograph
  3. Two additional identical and recent passport-size photos (taken within the last 6 months), printed according to ICAO international standards. White background.
  4. Signature authentication (only if one parent is not an EU citizen, including British citizens). The signature must be authenticated as shown on this page.
  5. The child’s original previous Italian passport, if available (even if expired or damaged), or a copy of another identity document of the child, if available. After processing, it will be returned together with the new passport.
  6. Photocopy of the passport or identity card of both parents/guardians (no other types of documents are accepted).
  7. In case of loss or theft of the child’s passport: form ETD2 and a copy of another identity document (if available).
  8. Printed proof of payment made by bank transfer, as indicated above.
  9. TwoPRE-PAID SPECIAL DELIVERY ENVELOPES” up to 500 g, with no expiry date, to be purchased at any post office:
    • The first envelope must have the address where you wish to receive the passport.
    • The second envelope should be used to send all documentation (including the first envelope) to the following address: Consulate General of Italy, Passport Office, 83–86 Farringdon St., EC4A 4BL – London.

If the required criteria are not met, or in the case of incomplete documentation, the application will be rejected and returned to the sender.


MISSING SIGNATURE OF ONE PARENT

  • If it is not possible to obtain the consent of the other parent, the applicant is invited to contact the Social Assistance Office by sending an email to sociale.londra@esteri.it.
  • In the event of the death of one of the parents, it will be necessary to submit the original death certificate, legalised and translated as described here (only if it has not already been registered with the relevant municipality in Italy).
