Child travel consent form for Italian minors under the age of 14

Italian citizens under the age of 14 who travel internationally without their parents must present an Accompanying Declaration (Dichiarazione di Accompagnamento) to border authorities together with their Italian passport or Italian identity card. This document is issued by Italian Embassies, Consulates, Police Headquarters (Questure), and Police Stations (Commissariati). It records the parents’ consent for the child to travel without them and authorises one or more designated accompanying adults.The declaration is valid for a maximum of six months from the date of issue and it may only be used for one round trip.

The Consulate General of Italy in London may issue an Accompanying Declaration only if all of the following conditions are met:

• The child is an Italian citizen.
• The child is under 14 years of age.
• The child will travel using an Italian passport or Italian identity card. For dual nationals travelling on a foreign passport, the rules of the issuing country apply. For Italian-British dual nationals travelling on a UK passport, please refer to the relevant guidance page.
• The child normally resides within the Consulate’s jurisdiction and is physically present there when the application is submitted.
• The child will not be travelling with either parent.
• The journey is international, involving more than one country.
• The accompanying person will be:  a transport company (e.g. ITA Airways, Ryanair, etc.), and/or at least one individual aged 18 or over.

Please note: An Accompanying Declaration does not guarantee that an airline or transport company will accept an unaccompanied minor. Parents should check directly with the carrier whether it offers an unaccompanied minor service.

Parents whose child meets the above requirements must follow one of the following procedures.

1. Both Parents Are Italian Citizens, EU Citizens, or Non-EU Citizens Normally Resident in Italy

If both parents are either citizens of an EU Member State; or non-EU citizens who are normally resident in Italy, they may apply free of charge by post.

The following documents must be mailed to the Notary Office:

1. Photocopies of both parents’ identity documents (EU identity card, EU passport, or Italian residence permit, if applicable).
2. A photocopy of the child’s Italian passport or Italian identity card that will be used for travel, including the pages showing the parents’ names and the issuing authority.
3. The Accompanying Declaration Application Form, completed and signed in original by both parents. If the parents live separately, they must sign the same original form in turn before it is sent to the Consulate.

Once the documentation has been received and verified as complete and correct, the Office will send the Accompanying Declaration electronically to the applicants’ email address. Parents must then forward it to the accompanying adult, who should present it on their smartphone to the relevant border authorities.

1. One Parent Is a Non-EU Citizen Who Does Not Reside in Italy

Non-EU citizens living outside Italy are not exempt from the requirement to have their signature officially authenticated. They must therefore book an appointment with the Notary Office and submit the application in person.

To request an appointment, send an email to notarile.londra@esteri.it  including:

• the full name and nationality of the non-EU parent who will attend the appointment;
• the child’s full name, date of birth, and place of birth;
• the planned date of travel and the countries involved, including any transit or stopover countries.

If the request is accepted, the Office will reply with an appointment. The non-EU parent must attend and bring:

1. the appointment confirmation email;
2. a valid identity document;
3. photocopies of both parents’ identity documents;
4. a photocopy of the child’s Italian passport or identity card (including the pages showing the parents’ names and the issuing authority);
5. the Accompanying Declaration Application Form, printed, signed by the EU parent and not signed by the non-EU parent, who will sign it at the Consulate;
6. a UK-issued debit card for payment. The fee is the amount specified under Article 24 of the Consular Fees. Please note that the Consulate accepts only UK-issued debit cards. The following are not accepted: contactless payments, Apple Pay, credit cards, Italian debit cards.

On the day of the appointment, the Office will email the Accompanying Declaration to the applicants. The parents must then forward it to the accompanying adult, who should present it on their smartphone to the border authorities.

If the non-EU parent is unable to attend the Consulate, they may instead: sign the Accompanying Declaration Application Form before any UK notary public and have the notarised document apostilled by the UK Legalisation Office (many notaries can arrange this service).

The notarised and apostilled form may then be submitted using the postal application procedure described in Section A.
