Consent forms for issuing an identity document for an Italian minor at another office

The Notary Office is only responsible for authenticating the signatures of non-EU parents on consent forms for the issuance of passports and identity cards for Italian minors by offices that do NOT belong to this same Consulate (Police Headquarters, Police Stations, Municipalities in Italy, and other Italian Consulates and Embassies).

Please note:

• The consent form is NO longer required for issuing Italian identity documents to parents with minor children. For more information, see the relevant page.

• Authentication of the signature on the consent form is NOT required for parents who are nationals of EU member states. Therefore, if the document is to be issued by offices other than this Consulate (Police Headquarters, Police Stations, Municipalities in Italy, or other Italian Consulates and Embassies), EU parents do NOT need to appear at the Consulate. It is sufficient to send by post to their minor child a copy of their European identity document along with the “consent form.” This must be printed, fully completed, and signed in original.

Only non-EU parents of Italian minors whose identity documents are to be issued by offices NOT belonging to this Consulate may request an appointment to have their signature authenticated on the consent form at the Notarial Office.

Non-EU parents who are unable to attend the Consulate may have their signature on the consent form authenticated by any UK public notary, and then have the document apostilled by the Legalisation Office (many notaries also offer this service). A non-exhaustive list of notaries known to this Consulate is available here. The completed document must then be sent in original by post to the minor child for whom the document is to be issued.

BOOKING PROCEDURE AND COST OF THE SERVICE

To proceed, you must send an email to: notarile.londra@esteri.it including the following information:

1. First name, surname, and nationality of the non-EU parent requesting authentication of their signature;
2. First name, surname, date, and place of birth of the Italian citizen whose document is to be issued;
3. Certified email address (PEC) of the office issuing the document (available on their website);
4. Date on which the document is expected to be issued.

Once the request has been verified, the Office will respond by offering an appointment at the Consulate.

At the appointment, only the non-EU parent must attend and bring:

1. The appointment confirmation email provided by the Office;
2. A valid identity document;
3. The Atto di Assenso “consent form” completed but NOT signed;
4. A UK debit card for payment. The cost of the service is as indicated in Article 19 of the Tabella dei dirittti consolari (Consular Fees Table). Please note that payments at the Consulate can only be made by UK debit card (no contactless, no Apple Pay, no credit cards, no Italian debit cards).

USE AND VALIDITY OF THE CONSENT FORM

On the day of the appointment, a scanned copy of the consent form and the non-EU parent’s document will be sent to the chosen Office and to the applicant. The consent form is valid for six months from the date of issue.
